For identifying information, please enter the following:

• Enter your 9 digit Participant Number (located next to your name and address on the form you received in the mail). Click on the "Questionnaire Example" to help you locate your participant number. You will need to enter this participant number each time you log in.

• Enter the first three letters of your last name.

• Enter your Date of Birth. Please enter the birth year as four digits (i.e. 1956)

It will take approximately 10 minutes to complete the Juror Qualification Questionnaire.

You will be prompted to provide an email address and opt-in for text messaging. It is important that you provide this information as the Court may need to contact you urgently, including to cancel your jury service.

Login to check your Status or Reporting Instructions. Once you are logged in, click on the tab labeled Status / Reporting Instructions. If qualified, you must check your reporting instructions after 7:00 p.m. the day before the report date on your summons. Jurors who live 80 miles or more (one-way) from the courthouse must check after 11:00 a.m. the day before they are to report. It is important to check your reporting instructions because not all jurors will be required to report.
