In order to complete your juror questionnaires or receive your reporting status:

• (1) Enter the 9 digit Participant Number, (2) Enter the first three letters of the last name, and (3) Enter the date of birth.

• If Completing Questionnaires: It will take approximately 20 minutes to complete the Questionnaires. Your submissions must be completed in one sitting, and your responses will NOT be retained if you exit before you complete the entire Questionnaire. Please be sure you have sufficient time to complete before you begin.

• If Completing Checking Reporting Status: Once you are logged in, click on the Status/Reporting Instructions tab.
